NURS6626 Care Coordination Leadership
Learners propose a project for change in a healthcare setting, aligning care coordination resources with community healthcare needs. Learners apply project management best practices to influence ethical practices and support positive health outcomes in the delivery of safe, culturally relevant care in compliance with applicable regulatory requirements. In addition, learners identify ways in which the care coordinator leader supports collaboration between key stakeholders in the care coordination process. Learners are required to complete 50 practicum hours. For MSN learners only. Special permission is required for registration. Cannot be fulfilled by transfer or credit for prior learning.
