NURS6622 Care Coordination Structure and Process
Students identify and analyze gaps in practice for a specific population and the barriers affecting the care coordination process. Students create a strategy to resolve the gaps through enhanced care coordination process and interprofessional collaboration to improve healthcare outcomes. Students are required to complete 50 practicum hours. For MSN students only. Special permission is required for registration. Cannot be fulfilled by transfer or credit for prior learning.
